When the Government (e.g., Army, Navy, Air Force), other end clients (e.g., defense, civil aviation, light rail, heavy rail including PTC) or their representatives attend our safety courses, it is not uncommon for them to ask:
"If you were us, what would you watch out for?"
They ask this question even in the case of prescriptive contracts. There are a few reasons why:
- One is that they are often overwhelmed by the prime contractors' detailed safety analyses.
- Another is that they often have limited time to thoroughly review these submissions.
- But, more importantly, they may lack in-depth knowledge in system safety and software safety, and are concerned about the pedigree of the prime contractor's safety engineers and their submissions.
Since there is no certification of system safety and software safety engineers in the U.S. and Canada, the knowledge and expertise of these people varies enormously and, as a result, many poor safety analyses and documents continue to be produced. We have seen many situations where safety documents have been reviewed and signed off by others (sometimes all the same day) yet these documents/analyses were sadly lacking. For example, we have seen utterly horrible SSPPs, hazard logs and fault tree analyses that should be flat-out rejected. We have also seen situations where safety analyses are not being produced in sync with design (e.g., Common Cause Analysis, O&SHA).
Many people are interested in knowing, beyond the deliverable documents themselves, what they should watch out for.
There are many situations where system safety documents/analyses should never be accepted due to the fact that:
- important elements are missing, misrepresented, or
- given insufficient attention thus compromising safety.
Hazard Logs are one example. There have been many horrible ones produced that should be flat-out rejected.
Fault Tree Analysis is another example. Perhaps you have seen our 28 pass/fail criteria.
