FAILURE MODE AND EFFECTS ANALYSIS (FMEA)


A Failure Mode and Effects Analysis is a procedure by which each potential failure mode in a system is analyzed to determine the results or effects thereof on the system and to classify each potential failure mode according to its severity.

Believe it or not, most people do not know what a FMEA is.  

An FMEA, for instance, DOES NOT contain failure mode probabilities.  If someone or some standard has defined it that way, they are wrong.  

​SAE ARP4761 defined FMEA very wrong (for more than this reason) !
